Hmm, on this shot I believe I used a single studio strobe with a 20 degree grid. The subject was facing pretty much straight into the light, which was to the left of the camera.

Sort of. I have shot multiple images at different exposures in order to manually composite them to achieve the effect. It's particularly useful in balancing muliple light sources with daylight.
